UPMC Altoona, in partnership with the James E. Van Zandt Veterans Medical Center, has launched a nurse liaison program. The program is aimed at helping veterans get the help they need, from admission to discharge. Reportedly the program will help in coordinating care involving physical therapy, prescriptions, and follow-up appointments. The program began three years ago at UPMC Harrisburg. Altoona is the second location in Pennsylvania to offer the program.
